The HAC Reduction Program (HACRP) allows CMS to adjust payments to hospitals that rank in the worst-performing 25 percent of all  hospitals with respect to HAC quality measures. As set forth in the Affordable Care Act, these hospitals may have their payments reduced to 99 percent of what would otherwise have been paid for such discharges. This typically amounts to millions of dollars.

For FY 2016, the Total HAC Score is based on data for four quality measures grouped into two domains:
  • Domain 1 – Agency for Healthcare Research and Quality (AHRQ) Patient Safety Indicator (PSI) measure:
    • PSI 90 Composite
  • Domain 2 - National Healthcare Safety Network (NHSN) Healthcare-Associated Infection (HAI) measures:
    • Central Line-Associated Bloodstream Infection (CLABSI)
    • Catheter-Associated Urinary Tract Infection (CAUTI)
    • Surgical Site Infection (SSI) – colon and hysterectomy
