Is it hemorrhagic or ischemic?

 

Where is the site of the hemorrhage? (Hemisphere - Brain stem - Cerebellum - Intra-ventricular- Multiple localized )

 

Is it embolic, due to thrombosis or stenosis ? 

Probably the most important question to answer is WHICH SIDE is the stroke on, L or R ?

 Also always remember to document the sequelae such as  hemiparesis, hemiplegia, aphasia, dysphagia, etc
