Incision & Drainage

Incision and drainage of skin and subcutaneous tissue (or closure of incision of skin) may not be something that hospitalists perform. However, the documentation that they provide will help with the accurate capture of patient information and facilitate the coding process.

In fact, it is a very simple documentation that makes sense. Basically there needs to be specificity around the site (which arm, scalp, neck...) and the DEPTH of the incision, whether it involves the skin or the subcutaneous tissue and fascia as well. If there is drainage, it would need to be documented.
