Glasgow Coma scale

Glasgow Coma scale ICD-10 codes will provide information on measurement scales that could not be reported with ICD-9-CM. The Glasgow Coma Scale includes three tests used to report criteria such as open eyes, verbal responses and motor responses. When individual scores for these three items are documented, ICD-10-CM codes would be assigned and reported.

Hepatic encephalopathy

Hepatic Encephalopathy ICD-9 has one code to describe hepatic encephalopathy. Because many patients may have hepatic encephalopathy and be chronically on lactulose, ICD-10 creates two codes to delineate the conditions based on severity: one for those “without coma” and another for those with significant symptoms (described as “with coma” in ICD-10). ICD-10 leaves the determination of what symptoms of hepatic encephalopathy constitute “with coma” to the physician. So remember WITH Coma WITHOUT Coma

eCQMs

What are eCQMs? eCQMs are quality measures that were developed specifically to allow an electronic health record (EHR) system certified to Office of National Coordinator (ONC) standards to calculate, export, and submit the measure data. There are currently 29 available eCQMs, 28 of which are applicable for the Hospital Inpatient Quality Reporting (IQR) Program. Hospitals must successfully submit data for at least four of the eCQMs to meet program requirements for the Hospital IQR Program. Any hospital that satisfies the electronic reporting requirement for the Hospital IQR Program will also be considered to be in compliance with the submission requirements for eCQMs under the EHR Incentive Program. As we adopt more electronic workflows, we will need to become more adapt at using the EHR to directly capture quality data
