Chronic Kidney Disease

Documentation for CKD in ICD-10 is not too different than ICD-9:

- Identify the stage (ex: stage 1, 2, etc.).
- Note the dialysis status.
- Detail any underlying or associated conditions (e.g., diabetes, hypertension, etc.).
- Mentionif there is  kidney transplant 

As a reminder, The stages of CKD (Chronic Kidney Disease) are mainly based on measured or estimated GFR (Glomerular Filtration Rate). There are five stages but kidney function is normal in Stage 1, and minimally reduced in Stage 2.

The KDOQI stages of kidney disease are:

Planning for endstage renal failure.
